How do we record our Information, Advice and Guidance?

We currently offer in house Information and Advice but we do not have a proper system of recording this process although we signpost the clients and make follow up phone calls, where appropriate. We are having an increasingly high demand for this type of service and we would like to be able to offer this service as part of our core activities and want to ensure that we follow the correct procedures and keep all the proper records. To ensure that we do this the correct way, a member of staff is now doing an IAG NVQ level 3 course. Is it possible for you to send me templates of forms for recording clients' interviews and follow up procedures, referrals, the right policies and procedures to have in place, etc.
